     GENERAL MEDICINE CASE SHEET(june14) CHIEF COMPLAINTS: A 35 year old female, fertilizer factory worker by occupation came to the opd with chief complaints of altered sensorium,lack of energy,fever and loss appetite for 3days,vomiting frequently. HISTORY OF PRESENT ILLNESS:                   Known case of hypertension()..  and recently diagnosed with severe diabetes   and hypersalivation fever-low grade. She quit her job 15 days before the development of symptoms and attended to a function 3days before the development of symptoms. HISTORY OF PAST ILLNESS: k/c/o :HTN Not k/c/o: diabetes  surgical history: umbilical hernia  PERSONAL HISTORY: Diet:mixed Appetite:decreased Bowel and bladder mixed: normal Addictions: none FAMILY HISTORY: normal  GENERAL EXAMINATION : Pallor:not seen Cyanosis:not seen Icterus:not seen  Lymphadenopathy:not seen Clubbing:not seen Oedema:not seen VITALS : Temperature: febrile  Pulse rate: 100/min  Respiratory rate : 22/min  BP: 140/90 SpO2 : 94% SYSTEMIC EXA

                GENERAL MEDICINE CASE (09-06-22) DATE OF ADMISSION - 08/06/22 CHIEF COMPLAINTS: -  Patient aged 28years was bought to casuality with giddiness and headache and decreased reaponsivness. -History of ratpoison consumption 4days back and discharged back. HISTORY OF PRESENT ILLNESS -Pt has h/o zinc posphide poisoning on 3rd june 2022 and pt was admitted on 3rd june and was treated conservatively and was discharged on 6th june. -Pt was normal on 6th and 7th june. • On 8th june Pt had headache and decreased appetite and 1 episode  of fever, no chills  -Then pt went to bed and had history of difficulty in speech and altered behaviour. -No h/o of seizures and vomitings,focal neurological defecit. HISTORY OF PAST ILLNESS  •NO H/O fever, pain abdomen, vomitngs, abdomen distension, pedal edema. PERSONAL HISTORY • Mixed diet • Appetite Normal • Sleep adequate • Bowel and Bladder movements regular. TREATMENT HISTORY    • Not k/c/o diabetes, HT, CAD , asthma, TB. •NO H/O chemo /radiat
